Youth in Transition Grants
Grants in this category will provide a range of services to support youth who are or have been in care, and have received Child Intervention Services. These grants will support youth as they transition into adulthood.

Grant funds must be directed to:
- youth 13-22, who are receiving or have received Child Intervention Services;
- youth at-risk which are defined as those who face the challenges of transition to adulthood, without appropriate family and support networks, educational and employment opportunities, or adequate food clothing and shelter; and
- a minimum of 10 youth who have received Child Intervention Services.
