Please review the Criteria page to ensure your program or project fits within the mandate of Alberta Human Services.

• Download, complete and sign the three forms listed above.
• Remember that you are applying for only one year of funding.  Note: when entering the grant amount on the forms, type the numbers only (no decimals) and hit “Enter”.
• Save the completed documents on your computer in the event that changes may be needed.

Please include the following with your package:

• Proof of filing with Corporate Registries.
• At least one letter of support from a partnering organization or community group.
• Supporting documents you deem appropriate.
