To begin filling out the form, click on the first field of the form. You can then use your mouse to click and type the required information, or press the TAB key on your keyboard to advance from field to field.
Before printing your completed form, press the TAB key to exit the last field of the form. Then click the printer icon in the upper left corner of the form to print 2 copies. Please ensure all information entered on the form has printed correctly.
After typing the appropriate information into the form, be careful not to use your browser back button before printing your form, as the form will reset itself and all information you have typed will be lost. You can save your form by selecting File, Save As from your browser menu. You can then re-open the form by double-clicking on the form name or opening Acrobat Reader, then retrieving the form.
If you prefer, you can print a blank copy of the form and fill it out by hand. Remember to make an extra copy for your files before mailing it.
If both you and the family member you are seeking voluntarily register, the Registry will contact you.
If both of you agree, professional staff will assist in the facilitation of reunion. This would include sharing general and identifying information and help in determining the best way to establish contact.
